Between 2006 and 2010, Realmuto was a three-sport athlete at Carl Albert High School in Midwest City, Oklahoma. He served as a shortstop for the baseball team, a quarterback for the football team and a power forward for the basketball team. During Realmuto’s high school years, both the Carl Albert baseball and football teams won their respective state championships. As a senior in 2010, Realmuto set a national high school baseball record with 119 runs batted in 42 games played (RBIs). He received All-American team selections from both the ABCA and the Louisville Sluggers. Realmuto also received the Bob Colon Scholarship, presented annually by the Oklahoman and Jim Thorpe Association to the top male high school scholar-athlete in the greater Oklahoma City area.

Realmuto received his first major league call-up on June 1, 2014. Realmuto made his major league debut on June 5, recording three RBIs in an 11–6 rout of the Tampa Bay Rays against Jake Odorizzi. Realmuto joined the Marlins for spring training in 2015 but was assigned to the Triple-A New Orleans Zephyrs to start the season. His first multi-home run game came on September 8, 2015 in a 6–4 win over the Milwaukee Brewers. At the end of the season, Realmuto was named the Marlins Rookie of the Year.

He maintained his offensive output until the start of the 2016 season. After scoring six runs, he was named the National League (NL) Player of the Week for the first week of the regular season. At the end of the season, Realmuto won the Jeff Konin Award, given annually to a player by the Miami Chapter of the Baseball Writers Association of America.

Realmuto began the 2018 season on the disabled list after sustaining bone spurs in his lower back. He suffered the injury during a spring training game against the New York Yankees. Realmuto received his first All-Star Game selection in 2018. Realmuto finished the season with a .277 batting average, 21 home runs, 74 RBIs and 74 runs scored in 531 at-bats. Realmuto became the Marlins’ Most Valuable Player in 2018. Realmuto was also the National League catcher selected for the 2018 Silver Slugger Award, given to the best offensive player at each position.

At the arbitration deadline on January 11, 2019, Realmuto agreed to a $5.9 million contract with the Marlins, including bonuses for each All-Star selection, Silver Slugger and Gold Glove Award. On November 3, Realmuto won his first career Gold Glove, joining Bob Boone and Mike Lieberthal as the third Phillies catcher to win the award. He received his second Silver Slugger, becoming the first Phillies player to win the award since Chase Utley ten years earlier. Realmuto was also named to the All-MLB First Team for December, the only Phillies player selected to the All-MLB team in 2019.

When the COVID-19 pandemic shortened the 2020 MLB season to 60 games, Phillies management wanted to make sure Realmuto would stay healthy during the shortened season. Realmuto officially became a free agent on November 11, 2020, when he declined the Phillies’ one-year, $18.9 million qualifying offer. On January 29, 2021, Realmuto agreed to a five-year, $115.5 million contract with the Phillies, the largest ever given to a major league catcher.

In Game 4 of the 2022 National League Division Series, against the Atlanta Braves, Realmuto became the first Phillies player and catcher to hit an inside-the-park home run in the postseason. In Game 1 of the 2022 World Series, against the Houston Astros, Realmuto hit a go-ahead game-winning home run in the top of the 10th inning. Realmuto won his third Silver Slugger and second Gold Glove award after the season.

On June 12, 2023, Realmuto hit for the cycle against the Diamondbacks, becoming the first Phillies player since David Bell in 2004 to accomplish the feat.
